Rates:
-Initial Consultation: Free
-Intake: $200, or contracted rate/co-pay amount with in-network insurance companies

-Session: $175, or contracted rate/co-pay amount with in-network insurance companies
-Cancellation Fee: (A fee may be owed if 48 hours cancellation notice is not given, exceptions only occur in the case of sickness, inclement weather, or in an emergency situation.)

*Please note, it is your responsibility to know and share with me what insurance coverage you have and if you also carry a secondary insurance. Disclosure of all insurance plans you are covered by is required previous to beginning treatment services.

*If engaging in telehealth video sessions, please check with your insurance company to make sure it’s a covered service and request details.

Self Pay:

Advantages of Self pay - You may choose to participate in a self-pay option if you do not want your insurance company to have access to your diagnosis or treatment record.

​Out-of-Network clients:

For those that have and wish to use their out-of-network benefits (due to having an insurance that I am not in-network with), if requested, I will happily supply you with the necessary documentation for you to submit claims for partial reimbursement (plan dependent). If you have a PPO or POS plan, you may have Out-of-Network Benefits (OON). Please call your insurance provider directly to determine if this applies to you, and what the claim submission process looks like, as well as at what percentage you may be reimbursed. *It is your responsibility to check with your insurance company to determine these details.

When contacting your insurance company, I suggest you ask the following questions:

  • Do I have out of network coverage?

  • What is my out of network coverage for Mental Health services?

  • Do I have a deductible to meet?

  • How much do you reimburse for mental health claims - codes: 90791, 90837, 90834, 90847 and telehealth video sessions?

Accepted forms of Payment:
I accept all major credit and debit cards, Health Spending Accounts and Flex Spending Accounts.

*Payment is due at time of service.
